24-32 Hours Per Week Providing Case Management to adults with intellectual disabilities and/or autism.
Case Management duties include regular visits, developing a person-centered plan, making referrals, coordinating services and funding, evaluating services, and documentation of case management services.
Work from home with a flexible schedule.
Qualifications: 4-year degree in Health and Human Services, Psychology, Social Sciences, or other related fields with 2 years experience preferred.
Requires State of Maine Driver’s license and ability to pass background checks.
