We have an anticipated opening for a Community Case Manager for areas in and around Franklin and Androscoggin Counties.
Case Managers support adults with autism and/or intellectual disabilities to identify goals, make a plan, coordinate funding, make referrals, and ensure satisfaction with their plan and providers.
Applicants must have a 4-year degree in health and human services, psychology, or other related fields with 1 year of experience in the disabilities services field.
