We are seeking a dynamic, independent, and caring community case manager. Community Case Managers support individuals with Autism and/or Intellectual Disabilities to identify and meet their goals, develop a plan to meet goals, refer and apply for resources, and evaluate how services are working. Case Management is a service reimbursed by Mainecare. Caseload is anticipated to include Franklin, Androscoggin, and Kennebec counties.

Qualifications: Bachelors degree in human services related field with 2 years of experience preferred. Candidates must be able to pass driving and background checks.
